Case Study: Rare Kidney Cancer in Ectopic Kidney Successfully Treated With Robotic Surgery – Doctors Share Details

An ectopic kidney is a congenital condition, meaning it is a birth defect in which the kidney is not located in its usual position. Instead of being positioned higher in the abdomen, the kidney remains in the pelvic region. This happens during fetal development when the kidney fails to move upward to its normal location while the fetus is growing.

This rare condition is known as an ectopic kidney. Some individuals with this condition may not experience any symptoms at all, while others may develop repeated kidney stones, urinary obstruction, or frequent urinary tract infections. In certain cases, an ectopic kidney can also increase the risk of developing a rare type of kidney cancer known as Renal Cell Carcinoma (RCC).

What Is Ectopic Kidney Cancer?

Diagnosing and treating cancer in an ectopic kidney is extremely challenging. Surgery becomes complicated because the blood vessels and surrounding structures of an ectopic kidney are often different from normal anatomy. Additionally, since the kidney is located in the pelvic region, surgeons have limited space to perform the operation safely.

Successful Treatment Through Robotic Surgery

Doctors at Medanta – The Medicity successfully treated this rare condition using advanced robotic surgery. The procedure involved the use of high-level diagnostic tools and robotic surgical technology designed to manage complex medical cases.

The case was handled by Dr. Puneet Ahluwalia, Senior Director and Head of Uro-Oncology and Robotic Surgery at Medanta Hospital in Gurugram.

Patient’s Condition

The case involved a 60-year-old patient who also had coronary artery disease, high blood pressure, and diabetes. During a routine medical examination for hypertension at another hospital, doctors detected an abnormality in the kidney.

Further investigation through abdominal ultrasound revealed that the patient’s left kidney was not located in its usual position. Instead, it was found in the pelvic region, confirming a case of ectopic kidney. Doctors also discovered a mass in the same kidney.

A contrast-enhanced CT scan was performed to better evaluate the tumor, which confirmed the presence of a renal tumor. Fortunately, the patient’s right kidney was in the normal position and functioning properly.

Test Results and Diagnosis

Doctors conducted a detailed metastatic evaluation to determine whether the cancer had spread to other parts of the body. The results confirmed that the tumor was localized and had not spread elsewhere.

A DTPA renal scan was also performed to assess kidney function. The test revealed that the ectopic left kidney was functioning very poorly. Based on these findings, the medical team explained the condition, treatment plan, potential risks, and expected outcomes to the patient’s family.

Robotic Surgery Procedure

Considering the complexity of the case, doctors decided to perform robot-assisted radical nephrectomy, a surgical procedure in which the affected kidney is removed.

The surgery was performed using the advanced Da Vinci Xi robotic surgical system. During the procedure, the patient was placed in a supine position while surgeons carefully navigated the pelvic anatomy. The surgical team gently moved the sigmoid colon to reach the ectopic kidney.

The kidney was carefully separated from surrounding structures. Surgeons then located the renal artery and renal vein, which were clipped and divided with precision. The entire kidney containing the tumor was removed successfully.

The robotic procedure required around two hours of console time, and blood loss during the surgery was minimal, demonstrating the accuracy and control provided by robotic surgical technology.

Recovery and Post-Surgery Care

Thanks to the precision of robotic surgery and the expertise of the medical team, the patient recovered smoothly without complications. The patient was discharged from the hospital on the second day after surgery.

A follow-up examination two months later confirmed that the patient was doing well. Post-operative tests showed stable serum creatinine levels, indicating that the remaining kidney had adapted well and was functioning properly.

Important Precautions After Kidney Removal

Patients who undergo kidney removal due to renal cell carcinoma must undergo regular monitoring to prevent recurrence. Doctors recommend:

  • Regular follow-up appointments and imaging tests to ensure the cancer does not return
  • Routine kidney function tests to monitor the health of the remaining kidney
  • Maintaining a healthy lifestyle and controlling conditions such as hypertension and diabetes

This case highlights how advanced diagnostics, multidisciplinary expertise, and robotic surgery can successfully treat complex and rare medical conditions. It also demonstrates how modern surgical technologies are improving outcomes for patients with challenging cancers.
