A rare medical mishap that occurred during a delicate medical procedure has come to light. Thankfully the situation was resolved when some foreign bodies left inside the patient during a medical procedure were removed by a team of doctors from Aakash Super Speciality Hospital, Dwarka, New Delhi, to which the case was referred in a critical condition.
A 62-year-old female patient was planned for a PTCA (percutaneous transluminal coronary angioplasty) at an external medical facility. A routine angioplasty procedure took a challenging turn when the catheter being used for the procedure inadvertently broke leaving a portion within the aorta – the body’s main artery. In such complex situations, a tool called a snare is commonly used to retrieve the broken object. To the patient’s bad luck, the retrieval process also took an unfortunate turn, as the catheter inside the aorta further broke into two pieces and even the snare broke leaving the fragment inside the aorta. The medical team worked hard to resolve the issue, however, realizing the complexity of the case, the patient was referred to Aakash Healthcare for further care.
Upon arrival at Aakash Healthcare, the patient underwent fluoroscopy to ascertain the precise location of the fragments inside the body. The situation was critical with three separate catheter fragments scattered inside the patient’s body – one inside the abdominal aorta and two in the patient’s legs.
“This is the third time in my medical practice that I have come across such a case,” said Dr. Ashish Agarwal, Director, Cardiology, Aakash Healthcare Super Specialty Hospital, Dwarka.
Explaining the tricky procedure Dr Agarwal added, “While removing catheter pieces from the legs was a tough task, however, the fragment stuck in the abdominal aorta with the snare was a major vascular surgery as we need to cut open the chest to access the aorta and carries a high mortality. Therefore, it was decided to remove the snare stuck inside the abdomen with another snare and within 5 minutes i could extract it following which the patient was handed over for further care,” said Dr. Ashish Agarwal.
“Another important factor was an angioplasty was contemplated and the patient was on blood thinners and had they cut opened the patient, he might have died of bleeding,” added Dr. Agarwal
Few days later, the patient underwent successful angioplasty at Aakash hospital and is currently stable.
The Cardiothoracic and Vascular Surgery (CTVS) team further removed catheter fragments from the femoral and popliteal arteries which was complicated by the mobility of the broken catheter pieces and required the use of advanced imaging techniques with a C-arm machine.
After an arduous four-hour operation, the medical team at Aakash Hospital emerged triumphant, having safely removed all fragments from vital blood vessels.
